New Pet? New Vet

    New to Westown Vet clinic?  Here are a few things you should be ready for:

Getting to know each other!

    - We will have you fill out a New Patient Pet Form here OR you can print out the one listed above and bring it in!

    - Bring in your previous health/ shot records if you have them! We will make copies if needed.

              (Don't have records? Call up your previous vet and have them fax it to (262) 798-2788)

What else to bring:

   - Fecal Sample: Puppies should have their fecal checked twice a year, adult dogs once a year at their yearly exam.

        Collect the sample in a clean container or a clean plastic bag. Keep refrigerated until going to the vet!
